Combination Therapy – Face and Neck
What is Combination Therapy?
As we mature, the neck undergoes degenerative changes due to aging, accumulated sun exposure, weight gain, and genetic predilection. The skin becomes lax forming multiple unwanted horizontal wrinkles. Vertical bands also become more prominent due repetitive contraction with talking, eating and facial expression. It is the contraction of these muscles in combination with aging and photo damage that lead to the appearance of the “turkey neck”. In addition, despite total body weight, the “double chin” also creates an undesirable appearance in both men and women and is fairly common.
Until recently, the only choice for rejuvenation of the neck and facial jowls was with invasive surgical intervention. However, surgery was often contraindicated in many patients due to high cost, lengthy recuperation times and poor mental or physical health. Today, we have the new options for the lower face using injectable fat dissolvers (Belkyra) along with laser treatments focused on energizing collagen (Fotona Dynamis).
Facial lines and overall sinking also present challenges as we mature as these both contribute to making us look older than we really are. Hyaluronic acid dermal fillers and Neuromodulator (Botox/Dysport) injections have revolutionized aesthetic treatment options as they are minimally invasive and require little or no down time. They are used in both the upper and lower face as well as in the neck regions.
Fortunately, with today’s new technology, these less invasive options are available and most often, the best results come for using them together called “Combination Therapy”.

Fotona Dynamis Laser
The Fotona Dynamis Laser works to heat collagen without disrupting the surface of the skin. It melts the collagen beneath the surface of the skin, encouraging it to reconnect and then form newer, tighter collagen once again. After a series of 3 to 4 treatments 4-6 weeks apart, patients enjoy an overall improvement with less skin laxity in the lower face and neck. The laser treatment itself requires no patient preparation and is virtually pain free with no down time required afterwards.

Probably one of the least desirable attributes is the “double chin”. Many patients have this genetic tendency despite not being overweight. Belkyra (Kybella in the USA) is a solution injected directly into the fat pad under the chin during a short clinic visit. On average, patients require 2 treatments about 2 months apart to dissolve this fat permanently. Patients can experience some discomfort and swelling for a few days after the procedure, but generally do very well and do not require time off from work.
Hyaluronic Acid Fillers
Facial deflation eventually becomes a challenge for all of us as we age. Injectable hyaluronic acid fillers are used to correct facial sinking of the entire face. When addressing the lower face, fillers can also be used to redefine the jawline, further improving the overall appearance of the lower face and neck. Syringe therapy is performed during a short clinic visit with a slight risk of post treatment bruising, but rarely requires post treatment down time.
Muscle relaxers are commonly used in the upper face to improve brow contour and smooth crows feet. Additionally, they are also used in the lower face, particularly with necklace lines and vertical bands visible in the neck. During a short clinic visit, tiny injections are made into these targeted areas with only a slight chance of post treatment bruising, but requires no down time.
